Master form for LDR notifications/certifications that are not available from offsite facilities
In a number of circumstances, hazardous waste generators must prepare LDR notifications or certifications, and they are having difficulty in finding appropriate forms to use. This form from McCoy's RCRA Unraveled has been developed to satisfy such LDR regulatory paperwork requirements. It has been formatted to allow direct data entry. You will be able to save this form to your computer and enter your data directly on your computer; however, if you only have the free Adobe® Reader software and not the full commercial version, you will only be able to fill in the data and print the formyou will not be able to save the data with the form as you can with the full version of Adobe® Acrobat. Of course, you can always print copies of the blank form and fill it in by hand.
Uniform Hazardous Waste Manifest and Uniform
Hazardous Waste Manifest Continuation Sheet
These forms from the Appendix to 40 CFR Part 262 in the federal hazardous waste regulations are EPA's hazardous waste manifest and continuation sheet. The actual manifest and continuation are 6-page forms, which must be obtained and distributed according to §262.21-§262.23.
RCRA Subtitle C Site Identification Form
This form (EPA Form 8700-12) is available for facilities that manage hazardous waste and are required by §262.12 to obtain an EPA identification number. A facility owner or operator must complete and submit the RCRA Subtitle C Site Identification Form for initial notification or subsequent revisions.
Lab Pack LDR Notification/Certification Form
This form can be used for lab packs containing hazardous wastes that will be incinerated under the alternative LDR treatment standards in §268.42(c). The form meets the information requirements specified in §268.7(a)(9) of the generator paperwork requirements.
RCRA Container Storage Checklist
Use of this checklist should help you reduce noncompliance associated with undocumented weekly container storage area inspections, open hazardous waste containers, improper container labeling, and inadequate or nonfunctional preparedness and prevention equipment in your container storage areas. This checklist was originally developed by EPA, but it has been reviewed for accuracy, streamlined, and improved by McCoy's seminar presenters.
Emergency Contacts Telephone Posting Form
Posting this completed form next to telephones in hazardous waste areas fulfills §262.34(d)(5)(ii) requirements for small quantity generators. The form was originally developed by the Colorado Department of Public Health and Environment.
Lead-Acid Batteries LDR Notification Form
This land disposal restrictions (LDR) form (Microsoft® Word format) can be used for entities that generate, collect, and/or transport lead-acid batteries to smelters under §266.80. According to information transmitted to us from EPA, LDR notification requirements for wastes that are shipped offsite without meeting the appropriate treatment standard are found in §268.7(a)(2). While the facility is not actually disposing of the batteries on the land, one-time LDR notification is essential to ensure cradle-to-grave management of the spent lead-acid batteries. Therefore, this form meets the information requirements specified in §268.7(a)(2) of the generator LDR paperwork requirements.
